Nurse Practitioners
Nurse Practitioners are able to diagnose and prescribe illness in a similar way to doctors and can care for patients with most acute illnesses. A large proportion of the practice's appointments which are available to book on the day will be with a nurse practitioner. Such appointments are ideal for patients with recent onset of symptoms and doctors are always on hand should their advice be needed.
Paramedics
Paramedics are a vital part of the general practice team. Working alongside our oncall team of Doctors and Nurse Practitioners they come out and visit housebound patients who have become unwell, in order to provide a quick assessment of their condition and support the clinicians on call in making the best decision for each patient's immediate and urgent care needs. When in the practice, they can also see and treat the same conditions as our Acute Illness nurses, namely new onset infections, rashes etc.
Clinical Pharmacists
Our Clinical Pharmacists, supported by pharmacy technicians, have an important role in prescribing safety, undertaking audits, responding to new advice and guidance around medication and ensuring patient medicines records are up-to-date when they come out of hospital. They also consult with patients around medication related issues and can prescribe/amend patients’ medication regimes as appropriate.

Assistant Practitioners
Assistant Practitioners support practice nurses with their daily work and carry out tasks such as dressings and pressure bandaging, diabetic checks, spirometry, some injections, phlebotomy (drawing blood), blood pressure measurement and new patient checks. They may act as a chaperone when a patient or doctor requests one.
Practice Nurses
Practice nurses are qualified and registered nurses. Practice Nurses can help with health issues such as family planning, healthy living advice, vaccinations, blood pressure checks and dressings. The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.
Acute illness Nurses
Acute Illness nurses can consult patients with day to day illnesses and prescribe certain medications. They can treat many conditions including throat/chest/urine/ear infections, rashes and other new onset illnesses.
Nurse Associate
Nursing Associates work with healthcare support workers and registered nurses to deliver care for patients and the public. Nursing Associates work across all four fields of nursing: adult, children’s, mental health, and learning disability.
Healthcare Assistants
Healthcare assistants support practice nurses with their daily work and carry out tasks such as diabetic checks, spirometry, some injections, phlebotomy (drawing blood), blood pressure measurement and new patient checks. They may act as a chaperone when a patient or doctor requests one.
Phlebotomists
Phlebotomists primarily see patients requiring blood tests, but some also undertake specialist INR tests for patients on warfarin or other blood thinning drugs. Phlebotomists are also trained to undertake ECGs and support the nursing team with other clinical tasks such as urine testing.
Mental Health Practitioners
Our experienced mental health team are on hand to provide those concerned about their mental health with an expert first consultation, establishing the next steps that are right for them, which might include referral to other agencies or medication as appropriate. Patients who contact us with a mental health related issue will usually be offered an appointment with one of our mental health team who can use their specialist knowledge in longer appointments to support the patient.
Physiotherapists
Our physiotherapists can provide expert assessment of patients with new muscle or joint issues, referring promptly and providing advice and guidance as necessary, without patients needing to consult with a GP. They can also provide advice and guidance for patients with ongoing physical joint and mobility issues.
Social Prescribers
Community/social prescribing is a way of linking patients to sources of appropriate, non-clinical support in the community. Our social prescribing facilitators are able to provide links for patients to opportunities that will support them with improving their physical and mental wellbeing. Senior Management Team
The Practice's senior management seeks to ensure that the right systems are in place to deliver, high-quality patient care, human resources, finance, patient safety, premises and equipment and information technology. They support the GPs and other medical professionals delivering patient services and manage our administrative team. They help to put processes in place to deliver extended services identified to enhance patient care, lead the practice in matters of clinical and information governance and work collaboratively to ensure the practice can deliver high-quality care to its patients.

Administration and Reception Team
We have a very dedicated and hardworking team of administrative staff whose responsibilities include dealing with all patient first contacts whether that be by telephone, electronically or face-to-face; summarisation of new patients' notes; handling of insurance and other medical claims paperwork and medicals; recall of patients for specific health checks, monitoring tests and medication reviews; medical typing and processing of patient referrals. Sometimes our staff may need to telephone patients to confirm certain details on behalf of the clinicians, such as prior to a referral being sent, or to pass on the response to an enquiry.
Dispensers
Our dispensing team are based at Homecroft and St Day sites, ensuring that patient who live more than a mile from a pharmacy can receive their medication from our dispensaries at those two sites. They need at least three working days between requesting medication and patients expecting to be able to pick up their dispensed items, please.
Community Midwives
If you are a patient of this practice, you can contact your midwife for non-urgent reasons by getting in touch with any of our sites. The receptionist will pass the details onto the midwives working with this practice. (For patients not registered with Carn to Coast Health Centres, please contact your own surgery who will also pass on your message to the midwives working with that practice). Messages are normally responded to by the midwives the same working day. For emergency midwifery support patients should contact the on call midwife via details on their care record pack.
Community Nurses
Community Matron - Our community matron is able to support patients with complex medical needs who are predominantly housebound by ensuring appropriate care plans are in place and that patients' medical conditions are appropriately monitored, involving the district nursing team as appropriate.
Counsellors
Psychological therapies are available and can be provided in the practice or at other sites. Please ask at/telephone Reception for a self-referral form. You can also go direct to the website of the provider, Outlook South West at https://www.cornwallft.nhs.uk/outlook-south-west/ where there is a wealth of self-help resources as well as the ability to refer yourself for support.
Health Visitors
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
